Your First Visit

Your first visit to Dental Connect is an exciting one, because together we will explore your mouth and teeth AND your thoughts to assess what we might achieve. We will provide a comprehensive dental examination and digital x-rays will be taken if necessary. It is a time when you can ask questions and advise us on your treatment wants and desires and from this we will create a treatment direction that is customised for you. The first appointment includes:

Check up or oral examination
Scale & Clean with fluoride
Digital x-rays (if needed)
Treatment plan and estimates (if treatment is required)

Prior to this first visit we ask that you fill out our comprehensive medical and dental history form. The information required on this form is essential for your care and individual treatment planning. If you would like to complete this questionnaire in the comfort of your own home then you will find this attached below. Please complete and bring with you to your dental appointment.
